Please Read Affiliate Agreement

This agreement contains the complete terms and conditions that apply to your participation as an affiliate of the Dental Depot . This is a legal agreement between you and dentaldepot.com. By clicking on the I agree at the end of this document you are affirmatively stating that you have read and understand the terms set forth herein and are affirmatively indicating your acceptance of this agreement and you agree to be bound by the terms hereof.

1. To begin enrollment you will submit a complete affiliate application via our web site You will be issued an office ID number by e-mail upon satisfactory completion of the on-line application and payment.
2. We will pay you a commission to 5% of the all sales  
3. Returned product or cancelled credit card will be deducted from the affiliates total.
4. Checks will be issued on January 15 for the prior year sales.
5. Checks that are for less than $5.00 will not be issued, except to the American Red Cross if so indicated. Amounts will be cumulative from year to year.
6. Remember for a limited time this is free!!!
